Program overview

What this program is

The Joint Restoration Program is Orthiva’s personalised, non-surgical path for chronic joint conditions. It is recommended only after an Orthiva Assessment — never chosen from a menu — and it is designed to reduce pain, ease stiffness and restore the movement a joint has lost over time.

Orthiva is educational first. This page explains the programme so you understand the approach before anything is recommended — because a programme is only ever suggested after a physician-led Orthiva Assessment, and is always built around your case, never chosen from a menu.

Our clinical philosophy

How Orthiva thinks about your care

Orthiva rests on a simple idea: lasting relief comes from understanding the whole person and the cause of a problem — not just quietening a symptom. That belief shapes how every programme is designed.

The whole person, not just the painful part

A painful knee or back sits within a whole body and a whole life. We look at posture, movement, daily load and general health — not the sore spot alone.

Restoring function, not only masking pain

The goal is to move better and do more, not simply to feel less for a while. Function — walking, stairs, sleep, work — is what we aim to restore.

Understanding the cause, not chasing the symptom

The same pain can have very different drivers. We try to understand what is actually behind it, so care addresses the cause rather than the alarm.

Built for the long term

Chronic conditions are managed over time, not switched off overnight. We plan for durable improvement and maintenance, and we are honest about the pace.

Who it's for

Who this program is for

The Joint Restoration Program is for people with a chronic joint condition — especially when earlier treatment has not held, and movement has narrowed over time.

This program is recommended for chronic conditions including:

How it works

The program journey

Every Joint Restoration Program follows the same staged arc over about 90 days — with a lighter Care Plan beyond it. Your doctor sets the exact lengths for your condition; here is the structure.

  1. 1

    Orthiva Assessment

Your Day-0 baseline: the Orthiva Score, measured by a physician. Fully credited toward your program.

  2. 2

    RESTORE

The intensive in-clinic stage — typically 3–4 weeks (your doctor sets the exact length for your condition, 2–5 weeks): doctor-administered classical therapies, personalised medicines, diet protocol. Day-30 progress check.

  3. 3

    REBUILD

Clinic visits taper; your home protocol and medicines continue, with scheduled reviews — building toward Day 90.

  4. 4

    Day-90 proof point

Your Orthiva Score is re-measured on the same instruments as Day 0. Progress is measured against your own baseline — never assumed. This is also the Dr. Era Ayurveda Assurance checkpoint.

  5. 5

    The Care Plan

After Day 90: a lighter maintenance layer — home protocol, periodic reviews, and a Day-180 re-measure — so the result holds. The stage most treatments skip.

Personalised, not packaged

What your plan may include

Every plan is personalised — the exact combination is decided only after your assessment, never chosen in advance. At a high level, a Joint Restoration Program may draw on:

Classical Ayurvedic therapy

Where appropriate for your joint, physician-selected classical therapies — chosen for your case, not from a fixed menu.

Personalised medicines

Internal Ayurvedic formulations matched to your condition and reviewed as you progress.

Diet & lifestyle guidance

Practical changes to reduce load on the joint and support recovery — food, activity and daily habits.

Movement & strengthening

Guided movement to rebuild the support around the joint and restore range, at a pace that suits you.

Structured follow-up

Scheduled reviews so the plan keeps pace with how your joint responds.

This is not a fixed package or a menu to choose from. What your programme actually includes is decided by your doctor, for your condition, only after your Orthiva Assessment.

Your plan is built after your assessment

The Orthiva Assessment comes first — a structured, physician-led evaluation of your condition. Only then does your doctor recommend a programme and design your plan.

Measured, not assumed

How progress is reviewed

Progress is measured, not assumed. Through the program your recovery is reviewed in three ways — and we are honest about what is, and isn’t, changing.

Clinical review

Your doctor reviews your symptoms and examination at intervals, and adjusts the plan to what they find.

Functional improvement

We track how your joint works in daily life — movement, stiffness and the activities that matter to you.

Your recovery journey

A realistic view of progress over time, with honest conversations about what to expect. We do not promise a specific result.

Your Orthiva Score

Progress is measured on your Orthiva Score — the WOMAC/ODI, pain, range-of-motion and function baseline recorded at your assessment — re-measured at Day 90 and Day 180.

Straightforward

Fees, honestly

Your program fee is quoted in writing at your assessment — standard rates, the same for everyone, with the program length and everything included stated upfront. Your assessment fee is fully credited toward your program. EMI options are available. There is no pressure to decide on the day — and no price that changes depending on who's asking.

Our written commitment

The Dr. Era Ayurveda Assurance

Your Day-0 Orthiva Score report carries a printed improvement threshold — the number we hold ourselves to. If your Score has not meaningfully improved by your Day-90 re-measure, and you have kept your side of the plan — attendance and home protocol, visible to you throughout — we repeat your program's treatment stages at no cost. One repeat, in writing, from Day 0.

We don't promise a result. We promise what we'll do if the numbers don't move.

‘Meaningful improvement’ uses the published thresholds of the medical instruments themselves (WOMAC/ODI/NDI/VAS), printed on your own report — never our judgement after the fact.

Evidence-based care

How Orthiva uses evidence

Evidence-informed care is more than following studies. Research points the way, but experience, your individual situation and honest reassessment decide what actually helps you.

Informed by research

We read classical practice against peer-reviewed research — including randomized controlled trials — measured with the same validated scores modern medicine uses.

Grounded in clinical experience

Evidence is applied through 18 years of physician-led practice and 16,000+ patients — what the research suggests, tested against what actually helps people.

Applied to your individual context

Studies describe groups; you are an individual. Your cause, severity, other health factors and goals shape what is appropriate for you.

Reviewed and adjusted over time

Evidence-informed care is not set-and-forget. Progress is reviewed honestly and the plan is adjusted to how you actually respond — including what is not changing.

Evidence-informed care

What the research shows

The classical therapies and formulations a Joint Restoration Program may draw on have been studied in peer-reviewed clinical research — including randomized controlled trials — measured with the same tools modern medicine uses: the WOMAC osteoarthritis index and the Visual Analog Scale (VAS) for pain.

  • Localised medicated-oil therapy (Janu Basti) with classical formulations, evaluated for knee osteoarthritis with improvement in pain and disability scores.[3][4]
  • Boswellia serrata (Shallaki) — a core anti-inflammatory in joint formulations — evaluated in multiple randomized, double-blind, placebo-controlled trials for knee osteoarthritis, with significant reductions in pain, stiffness and inflammatory markers.[8][9][10]
  • A multimodal Ayurveda regimen for knee osteoarthritis is currently being studied in a registered randomized controlled trial using validated WOMAC and imaging measures.[12]

An honest fit

Is this program right for you?

This program is built for chronic, ongoing conditions. It is not the right starting point for everyone — and we would rather say so, and point you toward the right care.

Suitable

  • Chronic knee or other joint pain lasting weeks or months
  • Osteoarthritis (early to moderate)
  • Joint stiffness and reduced range of movement
  • Joint pain that has not settled with rest or painkillers
  • Wanting a structured, non-surgical path before considering surgery

Not suitable

  • Emergency conditions or acute fractures
  • A joint that clearly requires urgent surgery
  • Signs of a joint infection (fever with a hot, swollen joint)
  • Serious, rapidly progressing symptoms that need specialist care

Your safety comes first

Clinical safety & when to seek care

An honest programme is also a safe one. Some situations need urgent, in-person medical attention — and some need a professional assessment before any self-guided step. Here is how to tell.

Seek prompt in-person care

This self-guided information cannot assess an emergency. If any of the following apply, please seek urgent medical attention rather than waiting:

  • A hot, swollen, intensely painful joint with fever — a possible joint infection
  • A joint that suddenly cannot bear any weight, or gives way completely
  • Severe pain or an obvious deformity after a fall or accident — a possible fracture
  • Rapidly increasing swelling, redness or pain over hours
  • Calf pain with swelling and warmth in one leg

When an assessment comes first

Self-management is not the right starting point for every situation. A physician-led assessment is the safer first step when:

  • Pain that is severe, constant, or steadily getting worse
  • Symptoms that have not improved with rest, movement or over-the-counter measures
  • Pain following a significant fall, accident or injury
  • New weakness, numbness or tingling in a limb
  • Symptoms alongside fever, unexplained weight loss or feeling generally unwell
  • Any condition where you are unsure what is safe to do next

Medical disclaimer: This page is educational and does not provide a diagnosis, a treatment plan, or medical advice for your individual case. It does not replace a consultation with a qualified doctor. A programme is only ever recommended after a physician-led Orthiva Assessment. Please do not stop or change any prescribed medication on your own, and seek prompt in-person care for any urgent symptom.
